Championship of Champions Entry Form

Entries close on September 20th at NOON.

NO ALCOHOL IN BARN AREA. NO LOW BODY SCORE HORSES. BRAND INSPECTION, HEALTH, AND COGGINS REQUIRED FOR ALL HORSES AND PONIES. NO EXCEPTIONS.

Race Requirements

Men's Relay - Must have been a paid member and attend 2 accredited races. Women's Relay - Must have attended one accredited race as paid member.

ACKNOWLEDGMENT AND ASSUMPTION OF RISK. I acknowledge that Indian Relay is an activity being held at tracks by the Horse Nations Indian Relay Council. I understand and agree that there are inherent risks of significant injury to myself and others by participating in the Indian Relay Races (herein after called “activity”). I understand and agree that participating in the Activity involves many risks and hazards, some of which are inherent to the very nature of the Activity itself; others of which may result from ordinary negligence including but not limited to falling for any reason, including uneven terrain, being airborne, jumping or collisions with other riders, horses, fixed or moving objects; as well as dangers arising from the excessive speed. Ordinary negligence or inexperience of other riders, from equipment failure or inadequate safety equipment; from uneven trails or changing trail/course conditions and weather conditions. I agree that if I believe conditions are unsafe, I will immediately discontinue participating in such Activity. I understand and agree that these risks may result in injury, harm or damage, including but not limited to sprains, torn muscles or ligaments, broken bones, strokes, heart stress, heart attacks, paralysis, disfigurement, death, or other forms of pain and suffering. I fully understand, voluntarily accept, and specifically assume responsibility for these risks of injury.

WAIVER OF LIABILITY AND INDEMNIFICATION. I agree to release and discharge from all liability and waive all claims, demands and actions against the Horse Nations Indian Relay Council (HNIRC) for any and all injuries, harms, or damages sustained in connection with my participation in the Activity, described above, resulting from the ordinary negligent acts or omissions of me; HNIRC, other participants I or any other person encountered while participating in the Activity. I agree to defend, indemnify and hold HNIRC harmless against any and all claims brought by anyone against HNIRC related to such injuries, harms or damages, including but not limited to payment of HNIRC, reasonable attorney’s fees and costs incurred in defending a claim demand or action waves herein.

RELEASE OF IMAGE AND/OR LIKENESS. I understand that the HNRIC and others commissioned by the Council will be taking photographs and making audio and video recordings of the Indian Relay and other related events. I hereby irrevocably consent to and grant the HNIRC and any persons authorized by HNIRC, the exclusive rights to the ownership and use of any and all photographs taken HNIRC or their authorized representative, that may contain my person, image and/or likeness; any and all audio or video recordings made by HNIRC agree that my image, voice, name, or likeness for any lawful purpose whatsoever.

ALL PARTICIPANTS ON TEAM MUST HAVE A SIGNED RELEASE ON FILE FOR SEASON OR TEAM OR RACER WILL BE DISQUALIFIED!!!!! IF YOU ARE ON THE TRACK DURING THE EVENT YOU BETTER HAVE A RELEASE SIGNED. ABSOLUTELY NO EXCEPTIONS.

Winners of accreditated races must have membership paid prior to racing in accrediated races to get free berth into championship of champions. Teams membership must be paid prior to racing in accredited races in order for the race to count toward the two required races for championship of champions entry.

Horses with poor body scores will not be allowed to race and they will be assessed prior to and during race. A lame or injured horse can be pulled prior to races each day by track veterinarian or official.
